Can't Re-certify online? Just follow the Mail-in Guidelines Below - Includes BA RE-CERTIFY EVERY 2 YEARS There are two different filing periods (and closing dates) per year for filing mail-in re-certifications: June 30th and December 31st. Consult the expiration date on your certificate to verify when you will be required to re-certify. Please do not wait for notification. Plan to submit your documents no later than the appropriate filing period expiration month. (June or December). |
PRINT OUT THE FORMS BELOW AS NEEDED
Please sign and date where required and submit your complete re-certification package by mail only. You will need to include the FOUR documents indicated below along with your payment and current (NOT original) photos of your work on one (1) client that you have had for at least 1-year. Please verify that all documents are included and that you have provided all required information. Incomplete packages will be returned to you for completion, and this could delay your re-certification.

2. Re-Certification Agreement (Regular) OR BA Re-Certification Agreement
3. Consultant Evaluation Questionnaire
4. Client Information Card (both sides required)
a) Client Information Card (front side)
b) Client Information Card (back side)
Please send a COPY of the original card (do not send the original) that reflects the client's history and shows that you have been keeping good records on the condition and progress of that client's hair. It may also show problems that have come up and how you dealt with them.
BEFORE YOU SUBMIT ...
Make sure to include good photos of your work! (Submit CURRENT photos, not the original ones.)
We need to be able to see THESE areas of the client's head in order to evaluate your work:
- Ear-to-ear part - how you positioned this.
- Side Parts - how you positioned them.
- Center Part, showing the number of locks from forehead to Ear-to-ear part.
- Front hairline, showing number of rows in Crown and Side sections.
- Back, showing straight parts for either 2 or 3 main sections.
- Back row count from nape of neck to Ear-to-ear part.
- Triangle at tip of R/L Back Sections
